Export Certificate Forms Under Review by USDA

The Department of Agriculture’s Food Safety and Inspection Service is accepting comments through May 21 on the proposed revision of an information collection for the Public Health Information System to include the FSIS standard export certificate forms 9060-5, 9060-5S, 9060-5A, and 9060-5B.

FSIS requires the use of form 9060-5, Meat and Poultry Export Certificate of Wholesomeness, for all meat and poultry exports and form 9060-5S, Fish and Fish Products Export Certificate of Wholesomeness, for all exports of fish of the order Siluriformes. FSIS also requires the use of forms 9060-5A and 9060-5B to capture additional statements or information that will not fit in the remarks section of forms 9060-5 and 9060-5S. In most cases, the PHIS will automatically populate the information collected on these forms from form 9060-6, Application for Export Certificate.
